NIDC Health Hub

NIDC is partnering with Montefiore Medical Center and Albert Einstein College of Medicine to launch NIDC’s Health Hub  – a Center to improve the physical & mental health of the communities we serve.  The environmental, “lived experience” factors that impact health represent 70% of the causative factors that impact health with the other 30% being genetic predisposition.  The Hub will use the “Social Determinants of Health” paradigm to inform programming in concert with our partners and participants.

Multiple modalities of connectivity to educate and support the community will be employed including: workshops, small group discussions facilitated by NIDC’s Social Worker, peer learning, health screenings, health and social service referrals, brief individual consultation with health professionals and interactive peer support. Workshops and materials will be provided in English and Spanish.

Programming will begin on a monthly basis at the Eastchester Gardens Community Center @ 3016 Yates Avenue.  Surveys and structured feedback will inform programming on an ongoing basis.  Our first three programs will be:

10/18 – Healthy Relationships – “What’s love got to do with it?” in support of October being Domestic Violence Awareness month.

11/15 Diabetes Prevention “You’re Too Sweet”

12/13 Coping with Stress and anxiety around the holidays  - “Don’t Sweat It”

As the program evolves, additional programming consistent with the SDOH will be added as well as additional partners and expanding the program to our other sites.
